Dr. Bartholomew is a resident psychiatrist at Gotham City's notorious Arkham Asylum for the Criminally Insane. When Batman learned that the Scarecrow was going to poison an underground river located beneath Arkham, he raced to stop him. However, he suffered the effects of Scarecrow's fear toxin and crashed the Batmobile. Bartholomew and his staff found Batman and his raving tirade as a sign that he had finally gone insane. Bartholomew attempted to treat Batman until he was able to convince the doctor that Scarecrow escaped his prison cell again.

Dr. Bartholomew was also one of the targets of former security head of Arkham, Lyle Bolton. Dr. Bartholomew was one of the persons responsible for Bolton's dismissal from Arkham after he was accused of torturing inmates. Dr. Bartholomew was not completely innocent. From a certain point of view, Bartholomew's methods can be attributed to what critics call the revolving door policy where criminal psychopaths are routinely committed then escape or serve their time, only to return to crime.
